Bridge Back Home (BBH) supports clients who are experiencing a crisis. Clients are referred by the Home Treatment Team (HTT), Assessment Admission Avoidance (AAA) and acute wards, and are supported in the community as they work towards their individual recovery goals. The service offers flexible assistance to people with enduring mental health needs through a seven-day shift rota. Support is focused on guiding clients through their recovery journey for up to, and sometimes beyond, an eight-week period. The role includes assistance with housing repairs, social inclusion, benefits, and emotional support.
The role also includes providing outcome-focused support, and collaborating across various services and sites as needed.
Working Pattern: While the majority of shifts are worked during daytime hours, you will be expected to work evenings and weekends in line with service needs. On occasions you may be asked to work on a weekend and longer than 5pm, this would depend on the need of the client and business. With reasonable notice, you may also be required to provide cover across our Medium Support, High Support and TILT services. |
